QUALIFICATIONS (CCSP)

* Persons must meet the level of care / functionally impaired criteria. An initial assessment is conducted by a registered nurse to determine appropriateness for services. Care plans are developed and approved by individual's private physician.

* Persons must be eligible or poten­tially eligible for Medicaid under program guidelines.

*Persons must be eligible or poten­tially eligible for Medicaid under program guidelines

* Persons can be adequately served through community based services within program cost limits.

*To Qualify for ALL OTHER SERVICES you must be 60 years old.
