What services does MFCOA offer?

SENIOR CENTERS / CONGREGATE MEALS

Hot, nutritious meals are served at Noon Monday through Friday at six area Senior Centers. The Centers also offer social activities, arts and crafts, exercise, indoor and outdoor games, vari­ous entertainment programs, and much-needed fellowship with peers.

HOME DELIVERED MEALS

Hot, nutritious meals are delivered Monday through Friday to homebound seniors.

COMMUNITY CARE SERVICES PROGRAM (CCSP)

Designed to coordinate existing, avail­able services for the elderly and/or functionally impaired in such a way that they can live inde­pendently in a safe, comfortable environment as an alternative to nursing home placement.

COMMUNITY BASED SERVICES (CBS)

Provides hot meals and transportation to seniors on a fee-for-service basis. Amount of charge is based on income.

QUALIFICATIONS (CCSP)

* Persons must meet the level of care / functionally impaired criteria. An initial assessment is conducted by a registered nurse to determine appropriateness for services. Care plans are developed and approved by individual's private physician. * Persons must be eligible or poten­tially eligible for Medicaid under program guidelines.

* Persons can be adequately served through community based services within program cost limits.

*To Qualify for ALL OTHER SERVICES you must be 60 years old.
